Grantee: HealthPoint CHC

Timeframe:   July 2015 - June 2018   |   Amount:   $370,000

HealthPoint’s project is designed to fill an identified unmet need to enhance primary care services for non-English and non-Spanish speaking patients by addressing language barriers and cultural differences. The project’s objective is to give these patients extra attention to help them understand, accept and engage in their plan of care. The project nurse provides the extra provider-patient time needed by the target group to help them accept and incorporate their plan of care with their cultural beliefs and values. The extra attention given by the project nurse aims to improve health outcomes in the target population by addressing patient-level variables that can cause a refusal of services, poor adherence to treatment, and delays in seeking care.
