Welcome Home Meal Delivery Program Expansion

PHPDA funds will be used for an expansion of the Welcome Home Meal Delivery Program, a nutritional intervention focused on reducing the rate of readmission to inpatient care of high risk populations and the analysis of derived data. Lifelong will complete an expansion of this program that will focus in South King County, mainly in the cities of Kent and Auburn.

County-Based Health Coverage for Immigrants

Timeframe: October 2016-September 2017 | Amount: $30,000

Grant funds will support NoHLA staff and a consultant in health economics in researching existing county-based programs (CBPs) that offer health coverage to uninsured immigrants, evaluating promising practices and analyzing how Washington and King County’s population, existing programs, potential financial offsets, and other factors would facilitate or create challenges for adapting such programs locally.

Patient Training Designs

A group of HEC students have partnered with IAF Northwest and Swedish Cherry Hill Family Medicine Residents to recreate the community organizing approach model by engaging and supporting patients in high disparity communities to become active agents in taking more control over their health.

Encompass Mobile Therapy Van

Encompass will create a mobile therapy vehicle in a modified Sprinter van that will be upfitted to contain a PCIT lab with a walled off play room, a one-way mirror and the sound and video equipment necessary to provide the program.
